If you are a current patient and have seen a provider here in the last year, feel free to click here. Our front office will contact you within 24 business hours regarding your request.

If your insurance has changed, please fill out the New Insurance Information form. You should call your new insurance company yourself to verify your mental health benefits and to find out directly whether you have any deductibles or co-pays so there are no surprises for you. Remember, if you haven’t been seen in over a year, you may need to be scheduled as a new patient or have an extended visit based on your provider’s discretion.
