If you are a current patient and have seen a provider here in the last year, feel free to click here or submit your request via our patient portal. Our front office will contact you within 24 business hours regarding your request. During normal business hours, you can call 770-422-2009 for our Marietta location or 770-667-1264 for our Roswell location to schedule a follow-up appointment. If you have trouble reaching us, you can email us at firstname.lastname@example.org and we will return your call asap. If your insurance has changed, please fill out the new insurance information form. You should call your new insurance company yourself to verify your mental health benefits and to find out directly whether you have any deductibles or co-pays so there are no surprises for you. Remember, if you haven’t been seen in over a year, you may need to be scheduled as a new patient or have an extended visit based on your provider’s discretion.
